Botox & Fillers

Overview

Wrinkle treatment options are increasingly abundant. There are numerous over-the-counter products, and people are also turning to their healthcare providers for longer-lasting options. Botulinum toxin type A (Botox) and dermal fillers are both long-lasting treatments. Each procedure can be used for wrinkles, but there are several differences between the two to consider.

Uses

Botox and dermal fillers alike may be used to treat wrinkles on the face. Each treatment is also delivered via injection. Still, both options have slightly different uses.

Efficacy

Is Botox effective?

Botox injections produce results for most people,

You’ll likely see noticeable effects within a week of the injection. Side effects are minimal, and most go away after a short time. You may not notice the full effects of Botox if you have certain conditions that prevent them. You’ll need to talk to your healthcare provider about all these potential risks ahead of time.

Once you receive the injections, you’ll be able to continue your daily activities without any recovery time. The effects of Botox last about 3 to 4 months. Then, you’ll need additional treatments if you want to maintain the results.

What is Botox?

Botox is a purified form of the botulinum toxin that is obtained from bacteria. Though it is deadly in larger amounts, the tiny, regulated amount of Botox given to correct wrinkles has been used safely for decades.

Botox works by blocking nerve signals in the muscles where it is injected. When those nerve signals are interrupted, the affected muscle is temporarily paralyzed or frozen. Without the movement of these selected muscles in the face, certain wrinkles may be softened, reduced, or even removed.

Botox and other treatments made with botulinum toxin are sometimes called neuromodulators or neurotoxins.

Treatments made with botulinum toxin are sold under the brand names Botox Cosmetic, Dysport, and Xeomin.

What can Botox correct?

Botox only works on wrinkles that are caused by muscle movement. These are known as dynamic wrinkles, and are often called “expression lines.”

The most common dynamic wrinkles that Botox can treat are lines on the upper face, such as the “11” between the brows, horizontal lines on the forehead, and crow’s feet around the eyes. These lines are caused by smiling, frowning, squinting, and other facial expressions.

Botox will not work on fine lines and wrinkles caused by sagging or loss of plumpness in the face. These are known as static wrinkles. Static wrinkles include lines in the cheeks, neck, and jowl areas.

Botox is not a permanent treatment. Repeated treatments are necessary for continued wrinkle-reducing effects. Most people find that the muscle-relaxing effect of Botox lasts for 3 to 4 months.

Who can/can’t have Botox?

Botox is generally considered an extremely safe treatment and there are very few exclusions. Pregnancy is a contraindication, as is myasthenia gravis and some other rare neurological conditions

What are the benefits of Botox?

Many people think of Botox injections as a cosmetic treatment only. While it helps eliminate wrinkles, the benefits of Botox go far beyond just providing aesthetic improvement.

  • Reduce facial wrinkles:  Let’s start with the benefits everyone thinks of first. Botox is an FDA-approved cosmetic procedure used to reduce dynamic facial wrinkles. These are the wrinkles that appear when you make an expression, such as squinting, laughing or frowning. Botox relaxes the muscles across your forehead, between your eyebrows and around your eyes to help you look younger.

  • Treat headaches and migraines:  If you experience persistent headaches, you know that migraine medication comes with dangerous side effects. Botox provides the relief many patients need with fewer possible side effects. While it’s uncertain how Botox helps migraines, it’s thought to work by blocking sensory nerves that send pain signals to the brain, as well as relaxing muscles that cause tension headaches.

  • Treat muscle spasms:  Botox was originally created to reduce eye spasms. This treatment method is still viable today and extends to other parts of the body as well, managing muscle stiffness and twitching in the elbows, wrists, fingers, ankles.

  • Reduce the effect of Bell’s palsy:  As a result of trauma to facial nerves, muscles weaken and droop, often on just one side of the face. Botox treats Bell’s palsy by relaxing unwanted muscle movements and reducing tension in hyperactive areas, creating more symmetrical facial expressions and reducing the signs of facial paralysis.

  • Relieve symptoms of an enlarged prostate:  By injecting Botox directly into the prostate gland, men with benign prostatic hyperplasia, or an enlarged prostate, can find relief from their symptoms. With long-lasting results and no need to take prescription medication, many men are considering this treatment option.

  • Reduce symptoms of overactive bladder:  Incontinence, frequent trips to the bathroom, and the sudden, urgent need to urinate can disrupt your daily life. Botox may help. When injected into the bladder muscle, it blocks nerves that trigger symptoms of overactive bladder, providing much-needed relief.

Botox injections are fast, affordable and non-invasive. The results also appear quickly – typically within a week or less – and last for three to four months or longer. So, whether you’re looking to improve your self-confidence or treat a serious medical condition, Botox injections could be right for you.

What are the risks and considerations?

Botox injections are relatively safe when performed by an experienced doctor. Possible side effects and complications include:

  • Pain, swelling or bruising at the injection site
  • Headache or flu-like symptoms
  • Droopy eyelid or cockeyed eyebrows
  • Crooked smile or drooling
  • Eye dryness or excessive tearing

Although very unlikely, it's possible for the toxin in the injection to spread in your body. Call your doctor right away if you notice any of these effects hours to weeks after receiving Botox:

  • Muscle weakness
  • Vision problems
  • Trouble speaking or swallowing
  • Breathing problems
  • Loss of bladder control

Doctors generally recommend against using Botox when you're pregnant or breast-feeding. And Botox should not be used in people who are allergic to cow's milk protein.

What should you ask your consultant before considering a treatment?

When getting any treatment, it’s extremely important that you feel safe in the hands of your practitioner. Never be afraid to ask lots of questions to ensure the job is being done properly and safely.

‘I would always suggest asking the qualifications of the practitioner and how long they have been doing the treatment, ‘Likewise, make sure the product being used is one of the proper brands.

‘In relation to the treatment itself, for cosmetic purposes, it is difficult to say who it’s ‘right’ for – this is always a matter of opinion and should be a joint decision between the patient and practitioner, based on their experience and your own wishes. If you ever feel coerced into having a treatment, then you should always take a step back and ask if it is right for you.’

What to expect?

Before the procedure:

Most people don't feel much discomfort during the procedure. But you may want your skin numbed beforehand, especially if your palms or soles are being treated for excessive sweating. Your doctor might use one or more of various methods available to numb the area, such as topical anaesthesia, ice and vibration anaesthesia, which uses massage to reduce discomfort.

During the procedure:

Botox injections are usually performed in a doctor's office. Your doctor uses a thin needle to inject tiny amounts of botulinum toxin into your skin or muscles. The number of injections needed depends on many factors, including the extent of the area being treated. Botox injections are usually done in a doctor's office.

After the procedure:

Do not rub or massage the treated areas for 24 hours. This may help prevent the toxin from spreading to a different area. You can return to your normal activities right after the procedure.

How much does Botox cost?

This depends on the clinic and practitioner. With a good, well trained and experienced doctor you’d expect to pay around 13,000 – 25,000 for a treatment for the full face. Beauty therapists are still unfortunately able to offer treatments, despite not being medically qualified, and prices are lower accordingly. This is all very well until something goes wrong!

What are fillers?

Dermal fillers, sometimes called soft tissue fillers, are substances designed to be injected beneath the surface of the skin to add volume and fullness.

Substances used in dermal fillers include:

  • Calcium hydroxylapatite, which is a mineral-like compound found in bones.
  • Hyaluronic acid, which is found in some fluids and tissues in the body that add plumpness to the skin.
  • Polyalkylimide, a transparent gel that is compatible with the body.
  • Polylactic acid, which stimulates the skin to make more collagen.
  • Polymethyl-methacrylate microspheres (PMMA), a semi-permanent filler

Each one of these is designed to treat different signs of ageing or other cosmetic issues.

The time they take to work, as well as how long they last, also vary. Some fillers last 6 months, while others last up to 2 years or longer.

People should discuss their individual needs and expectations with their doctor to determine what filler would be the best choice for them.

What can fillers correct?

Different types of dermal fillers are designed to treat varying signs of ageing. Depending on the filler selected, they may:

  • plump up thinning lips
  • enhance or fill in shallow areas on the face
  • decrease or remove the shadow or wrinkle under the eyes caused by the lower eyelid
  • fill in or soften the look of recessed scars
  • fill in or soften static wrinkles, especially on the lower face

Static wrinkles include those around the mouth and along the cheeks. These wrinkles are usually a result of a loss of collagen and elasticity in the skin.

What are the benefits of fillers?

  1. Immediate and visible results

Once you undergo a filler injection, the improvement in your facial contours become evident immediately. You don’t have to wait to enjoy the benefits of the treatment as you would with other, more invasive procedures.

Once the dermal filler is injected into the target area, it immediately adds volume and smooths out wrinkles and creases in your skin. As the filler settles under the skin, the full effect is visible in a matter of days.

If you’re in need of a last-minute boost to your appearance, dermal fillers are definitely the way to go!

  1. No recovery or downtime

Depending on the number of injections you’re getting at your appointment, a dermal filler treatment can be as quick as 10 minutes. You literally can have the procedure done during your lunch break then get back to your normal routine.

This non-invasive procedure doesn’t require you to take time off work because there's no surgery involved, which also means no post-op scarring or healing to deal with.

There may be some slight swelling just after the injections, but this subsides over the next couple of days as the gel settles under the skin.

  1. Minimal risk

Dermal fillers carry the risk of redness, swelling, and mild bruising at the injection site. These are side effects that gradually disappear over a few days.

Another risk is facial asymmetry, where one side of your face appears slightly different from the other. This risk is tied to the expertise and skill level of the person performing the dermal filler injections.

Our practitioners at Cleveland Family Health are well-trained and have the experience to ensure that facial asymmetry is one risk you need not worry about.

Severe complications from dermal fillers are rare. The outcomes are good when the procedure is carried out by a board-certified aesthetic practitioner who understands the risks and is trained to deal with any complications that may occur.

  1. Long-lasting results

How long the effects of your procedure last depends on the type of dermal filler used, the area of treatment, and the patient. Natural fillers typically last 6-18 months, and some synthetic fillers can last up to several years.

If you’re a first-time user, you may prefer the shorter duration option so you can see if you like your new look and would like to stick with it, or if you would like the injections done in different areas during your next visit.

The longer duration option may be better once you've settled on the look you prefer.

  1. Confidence boost

Dermal fillers improve your appearance in a very subtle way. You won’t leave our office a completely different and unrecognizable version of yourself. Your facial contours will be sharper, and your skin smoother.

In the hands of our experienced aesthetic practitioners, you achieve natural and amazing results.

What are the risks and considerations Dermal fillers?

Dermal fillers are considered to be safe but side effects can occur. The most common problems include:

  • skin rash, itching, or pimple-like eruptions
  • redness, bruising, bleeding, or swelling
  • undesirable appearance, such as asymmetry, lumps, or overcorrection of wrinkles
  • skin damage that causes a wound, infection, or scarring
  • ability to feel the filler substance under the skin
  • blindness or other vision problems
  • death of skin cells due to loss of blood flow to the area

The cost of dermal filler treatments varies and depends on the provider performing it, the area being treated and the type of filler selected. Fillers usually cost  INR 15,000 – 30,000 per ml in India.  Below are the average costs of different types of fillers:

  • calcium hydroxylapatite, such as Radiesse: 30,000 – 50,000 Rs
  • hyaluronic acid, such as Juvederm, Restylane, or Belotero: 15,000 - 30,000 Rs
  • polylactic acid, such as Sculptra: 50,000 – 75,000 Rs
  • Platelet-Rich Plasma or PRP: 40,000 – 55,000 Rs
  • polymethyl-methacrylate microspheres, such as Bellafill or Artefill: 50,000 – 75,000 Rs
  • Fat Grafting – 1,00,000 – 2,00,000

These costs may be more or less, depending on how much filler is used. Using less than a full syringe of filler may be cheaper than using a full syringe or more than one.

The provider may also charge additional fees for their professional services, office visit, or other costs.

What determines how much dermal fillers cost for dark circles?

The cost of dermal fillers for dark circles is determined by the following factors:

  • The qualifications and expertise of the person doing the therapy
  • The extent of the treatment area
  • The brand and quality of filler used
  • The type, amount and longevity of filler used
  • The technique used for the injection
  • The effort and time the procedure needs
  • The geographic location of the clinic

How are cheap dermal fillers different?

Fake and cheap dermal fillers are available online for free, often from China that has no manufacturing standards. Improper fillers can cause critical damage. So, it’s almost essential that you use only those from licensed suppliers and manufacturers. The risks involved in non-surgical cosmetic processes are greater in inefficient ones.

Quality dermal fillers are expensive as they guarantee effectiveness. So patients getting them can expect quality services in proper environments. The expense is involved in doing a lot of research, manufacturing quality products based on those research, providing brilliant services, meeting the needful safety measures and showing evidence of the efficacy and safety of their products.

What to expect?

Before the procedure:

Before the start of your procedure, your facial appearance and skin tone will be thoroughly examined by your medical professional. Strategic points on your face may get marked as the appropriate injection sites for the filler. Your medical professional may capture photographs of the soon-to-be treated area(s).

Your injection sites will be cleaned with an antibacterial agent. If you experience any pain at the injection site, a cold instrument will aid your skin along with anaesthetic ointment to numb the treated area.

This procedure is typically noted as being painless, and the injections are easily manageable for your medical professional. If you do experience pain, please inform your doctor to ease that right up for you.

During the procedure:

The injection itself takes a few moments per treated area. Your medical professional will inject, massage, and evaluate the area being treated. If needed, an additional filler may be used to make sure you are happy with the results and that it meets your aspirations. Depending on your personalized treatment, the process may range from 15 to 60 minutes.

After the procedure:

If you are satisfied with the results, leftover markings will be cleaned. An ice pack may be used to reduce swelling further and relieve any discomfort. Your treated area may feel sore for a day or two. You should not need medication.

How much do fillers cost?

Like Botox, the price of dermal fillers will vary hugely. At a good, medical clinic I would expect the cost per syringe of Juvederm to be 15,000 – 30,000 Rs. For larger volume, full-face treatments it’s expected to pay more. ‘Lower than this, one should definitely question the experience and quality of the product being used.’

How to choose between Botox and fillers?

The important things to consider when choosing between Botox and facial fillers are what kind of wrinkles you want to get rid of and how. Botox is the better choice for dynamic wrinkles as it will reduce their motion and visibility. These wrinkles occur when the face is in motion or making an expression. They are usually positioned in the forehead and around the eyebrows. Facial fillers are ideal for static wrinkles, which are visible even when your face is relaxed and making no expression. These areas are depleted and sunken in and will benefit from the fullness that facial fillers give them.

Knowing the difference between Botox and facial fillers and how they work matters significantly when seeking treatment for your wrinkles. Whichever one you choose, both forms of treatments are very efficient ways to recapturing and maintaining a youthful appearance.

Key differences

In summary, the differences between Botox and fillers are:

  • Botox : This freezes muscles to stop creases and wrinkles caused by facial expressions. These are typically found in the upper face, such as the forehead and around the eyes.
  • Dermal fillers : These use hyaluronic acid and similar substances to “fill in” or plump areas that have lost volume and smoothness. This includes wrinkles around the mouth, thin lips, and cheeks that have lost fullness. They may also be used on forehead wrinkles, scars, and other areas that need extra volume for a smoother look.
  • Botox results last 3 to 4 months. Dermal filler results vary, depending on which filler is used.

Because Botox and fillers are different substances designed for different uses, they can sometimes be combined in one treatment. For instance, someone may use Botox to correct lines between the eyes and a filler to correct smile lines around the mouth.

Take Away:

Millions of Botox and filler procedures are performed each year, and they have a good track record of safety.

Botox and filler procedures are very safe when performed by experienced board-certified dermatologists. Side effects occurred in less than 1 per cent of recipients, and most of these were minor.

Though Botox and fillers are minimally invasive, they still carry some risks. An individual should be aware of all possible risks and benefits before having these treatments.

Botox and fillers are not recommended for women who are pregnant or breastfeeding. People who have certain health conditions or who take any medications should discuss whether Botox or fillers are safe for them.

People should also discuss what Botox and fillers can realistically do for their appearance. Though they can enhance a more youthful look, the results are typically not as powerful as having a surgical procedure, such as a facelift.

Using a qualified medical provider, such as a board-certified dermatologist, plastic surgeon, or cosmetic surgeon helps ensure the procedure is done safely and properly. People should discuss the provider’s experience and training in dermal fillers and Botox before making a decision.
